Job Title: Mental Health Practitioner

Accountable to: The Lead Nurse Partner, Lead Pharmacist, Senior Management Team and the wider LMP Partnership

Salary: £33,730 - £48,510 per annum, pro rata

Job Summary:

As a Mental Health Practitioner (MHP) within Lancaster Medical Practice you will play an important role supporting patients, with mild to moderate mental health problems.
You will provide support to people suffering from various mental health conditions. The work involves helping the patient to access the most appropriate services, to recover from their illness or to come to terms with it in order to lead a positive life.
The role of the MHP will be part of the Advanced Clinical Team within the Multi-Professional Clinical Team (MPCT) and you will work closely with GPs, ACP’s and other Healthcare Professionals.

Job Title: First Contact Physiotherapist

Accountable to: The Lead Nurse Partner

Salary: £33,730 - £48,510 per annum, pro-rata

Job Summary:

The role will provide clinical expertise, acting as first-contact physiotherapist and making patient-centred decisions. Multi-disciplinary team working is key.
The role will consult with patients, without prior contact with their GP, to establish a rapid and accurate diagnosis and management plan, request and progress investigations to facilitate diagnosis and choice of treatment regime and then interpret and act on results to aid diagnosis and the preparation of management plans for patients.


This is a new role for the Practice and therefore, this job description may be reviewed periodically. A key part of the role will be to work with other clinical leads to grow and develop the role.
